If you were involved in a DUI/DWI incident in Maryland, the Motor Vehicle Administration (MVA) may require you to complete specific Driver Wellness & Safety forms before allowing you to keep or reinstate your driving privileges.


One of the most common forms is Form DCD-236 – Treatment Provider’s Report. This form is used by the MVA to receive official information from a licensed treatment provider regarding your substance use assessment, treatment participation, and progress.
